Got one from Heroic 4 (Breakthrough) on Belsavis, Imperial side. Dropped from a trash mob within the heroic. Many report that they come from the lockbox in the heroic and the reward box. I got 3 in 2 runs. 2 from golds, 1 from the champ. I did not turn in the quest so I do not know about the lock box.

I have got 4/4 doing H4: Breakthrough from the reward box and one more for killing the champ
Got mine from Freeing the Fallen at the end in the reward box.
Accept the quest Heroic 4: Breakthrough. Zone in, head down the right side and loot the chest near the end. Then kill the champion. Zone out, reset quest, repeat as desired. Very reliable datacron drops using this method.
