Same, hasn’t dropped from any other bosses for me either yet. Always Guid Patriarch, all mine were in normal mode. You have to do all bonus missions to get this boss.

Did not drop from bonus boss for me, only from Serevin. (SM)

in Story Mode the droprate is around 60-75% from bonus and Serevin. In Hard Mode it’s supposed to be a guaranteed drop from bonus (100%) and a 85-95% chance of dropping from Serevin. At least that is how the others seem to be set up
To get these you need to run the FP in SM or only HM?
I got one in sm.
Dropped off the bonus boss on my 3rd solo run of SM.
I got two to drop in HM, one bonus boss and the other from the last boss.
